All passport application forms can be downloaded from the Department of State website.

Please note that both letter and A4 size are acceptable formats.

Applying for a first time passport at the age of 16 or older:

  • completed form DS-11 and correct photograph

Applying for a first time passport/passport renewal for a minor under the age of 16:

  • completed form DS-11 and correct photograph

 Applying for passport renewal at the age of 16 or older:

  • completed form DS-82 and correct photograph

Statement regarding lost or stolen passport

Form DS-64 should be submitted along with form DS-11 when applying for replacement of a lost or stolen passport.

Statement of consent or special circumstances

Form DS-3053 should be used by a parent or guardian who needs to provide written consent for a minor’s passport application and is not able to appear in person with the child. The form must be notarized. It is possible to have it notarized at the Embassy (no fee) or at the ‘Administration communale’ of your place of residence in Luxembourg.


Effective January 1, 2016, the Department will no longer provide supplemental 24-page visa page inserts for U.S. passports. Instead, 52-page passport books will be issued at no extra cost to U.S. citizens applying from outside the United States. Renew your passport early to make sure you have enough blank pages for visa stamps from other countries. Some require two blank pages for entry and exit.

Passport Re-application 

Form DS-5504 should be used when requesting a data change for a previously issued, currently valid U.S. passport WITHIN one year from the date of issue.
